What Is a Night Guard?
A night guard is a custom-made oral appliance worn over your teeth during sleep. It creates a protective barrier between your upper and lower teeth, helping prevent damage caused by grinding or clenching (a condition called “bruxism”).
Depending on the severity of your bruxism and your specific symptoms, your night guard may also include design features intended to better distribute bite forces or help manage strain on the jaw joints and surrounding muscles.

Protective vs. Therapeutic Night Guards
Night guards fall into two general categories, depending on the treatment goals:
- Protective Night Guards are designed primarily to protect your teeth or dental work (e.g., crowns) from damage. Protective night guards absorb or redistribute the forces of grinding or clenching to help prevent wear and fractures.
- Therapeutic Night Guards are designed to protect, but also to help manage functional issues involving your bite and jaw joints. Therapeutic night guards adjust how your teeth come together during sleep in order to reduce strain on the jaw joints (TMJ) and surrounding muscles.
For many patients, simple protective night guards are enough. For patients with more advanced symptoms – such as jaw pain, clicking, muscle tension, or joint dysfunction – therapeutic night guards may be more helpful.

Depending on the purpose of your night guard (protective vs. therapeutic), the severity of your grinding or clenching, and specific issues with your bite, your dentist may recommend one of the following night guard designs:
Retainer-Style Night Guards
Common choice for straightforward protection
These guards are made of thin plastic materials and have a contoured biting surface designed to follow the natural grooves of your teeth (as the name implies, they look a lot like retainers). They’re used when the goal is protection without altering how your teeth fit together.
Flat-Plane Night Guards (also called Michigan Splints)
Designed for protection with added force balance
These guards may be made of the same thin plastics as retainer-style guards (for enhanced comfort) or may use thicker plastic materials (for added strength). Instead of having natural tooth contours, the biting surface in these guards forms a continuous flat surface along the entire arch.
This design is sometimes used for heavier grinding or when more even force distribution is needed. It can serve either a protective or therapeutic purpose, depending on how it is adjusted.
Anterior Bite Plane
Designed to help jaw muscles relax
In this design, a flat raised platform in the front biting surface ensures that only the front teeth make contact while the back teeth remain separated.
This type of guard is typically used for shorter periods to reduce muscle activity or for diagnostic purposes.
Anterior Repositioning Splint
Designed to guide jaw position
This appliance includes a guiding ramp on the biting surface that positions the lower jaw slightly forward when biting.
It is generally used with specific TMJ-related conditions.

How Long Will I Need to Wear a Night Guard?
Because bruxism is often a long-term condition, night guards may need to be worn for years – or even for life.
For that reason, comfort and proper fit matter. Unlike over-the-counter “boil and bite” products, which can be bulky or have a less-than-ideal fit, professionally-made custom night guards are designed specifically for your bite, providing more reliable comfort and protection over time.
And among the various types of professionally-made night guards, retainer-style night guards are likely to be the most comfortable option (provided they’re an appropriate option for your case).
How Long Do Night Guards Last?
It depends on the material and how heavily you grind:
- Night guards made of thicker plastic materials may last for years, but can still break or become difficult to keep clean;
- Night guards made of thinner and/or softer plastic materials wear down more quickly and should be replaced more regularly.
Retainer-style night guards are thin pieces of plastic. It makes them lightweight and comfortable to wear. But it also makes them more likely to accumulate scratches, other types of surface blemishes, and areas of distortion over time.
These issues create two problems for night guards:
- They become better homes for bacteria, which can cause odors, cavities, and gum disease. Eventually, no reasonable cleaning routine will be able to get rid of enough bacteria to keep your night guards truly clean – even if you see no obvious signs of dirtiness.
- They may no longer work as well as intended. Over time, subtle wear, distortions, and material fatigue can reduce how predictably your night guard manages biting forces. Even if the night guard appears intact, it may not be providing the same level of protection as when it was new.

So How Often Should I Replace My Night Guards?
To be sure that your night guards stay clean and keep working properly, you should consider replacing retainer-style night guards at least once per year.
