What Are Retainers?

Retainers are custom devices designed to keep your teeth in place after orthodontic treatment.

They come in several forms:

  • Hawley retainers – removable, with wires and a plastic palate
  • Fixed retainers – bonded to the back of your teeth
  • Essix- or aligner-style retainers – the ones that look like aligners.

Your dentist will recommend the type that’s best for you. The choice may depend on the stage of your treatment, your lifestyle, or other aspects of your case.

Do I Really Have to Wear Retainers for Life?

If you really want to keep your teeth from shifting, then yes, you’ll want to make retainers a part of your permanent smile care routine.

Teeth may look fixed in place, but they actually move, just slowly. Without retainers, everyone’s teeth shift naturally over time. Even if you’ve never had orthodontic treatment, you may notice gradual crowding (especially in your lower front teeth) or new spaces or angles.

And if you’ve had orthodontic treatment, your teeth may be even more likely to move.

But the good news is that aligner-style retainers are designed to be an easy part of daily smile care.

When you initially finish with your aligners or braces, your dentist will likely recommend that you wear your retainers full-time (except when you’re eating). But after one to three months, most patients can shift to wearing retainers only overnight.

At that point, retainer use is a lot like brushing your teeth: something most people can handle quickly after waking and before sleeping each day.

How Long Do Retainers Last?

It depends on the type of retainer:

  • Fixed retainers are designed to last for years;
  • Hawley retainers may also last for years, but their plastic parts can break or become difficult to keep clean;
  • Aligner-style retainers should be replaced regularly, more like toothbrushes.

Aligner-style retainers are thin pieces of plastic. It’s what makes them lightweight and comfortable to wear. But even with excellent care, thin pieces of plastic will tend to accumulate scratches, tiny pores, and distortions over time.

These issues create two problems for retainers:

  1. They become better homes for bacteria, which can cause odors, cavities, and gum disease;
  2. They may stop holding your teeth in the same place.

Yes, it’s possible for an aligner-style retainer to look fine after years of use. But looks can be deceiving.

First, there’s the hygiene problem.

If you’re wearing a thin piece of plastic every night for years, scratches and other surface blemishes will inevitably accumulate. These things are invitations for bacteria to set up shop. Eventually, no reasonable cleaning routine will be able to get rid of enough bacteria to keep your retainers truly clean – even if you see no obvious signs of dirtiness.

But worse, there’s the function problem.

Aligner-style retainers are thin pieces of plastic. And plastic things can change shape when heated.

For thin plastic devices – especially ones that get flexed daily as you place and remove them – mouth temperature alone can be high enough to cause changes in shape over time.

They may still look intact, but if your retainers are changing shape, they’re no longer working. Even subtle changes can mean your retainers are now:

  • Just moving with your teeth (no longer keeping them in place), or
  • Actively pushing your teeth toward their new shape.

Either way, it’s time for new retainers…

So How Frequently Should I Replace My Retainers?

To be sure that your retainers stay clean and keep working properly, you should consider replacing aligner-style retainers at least once per year.

You’ll find more and more providers offering aligner-style retainers on a subscription basis for this reason, with every-six-months replacements emerging as a popular option.

Some patients prefer even more frequent replacements for maximum peace of mind.

Won’t that be pricey?

It doesn’t have to be.
